Fluidnet Benefits

The following benefits are provided for our full-time colleagues:

Medical

Harvard Pilgrim Health Care Best Buy PPO. This robust PPO plan has a $1,000 deductible per member and $2,000 deductible per family which is 100% reimbursed to the employee by Fluidnet as incurred. Currently, Fluidnet pays 80% of medical insurance premiums.

Dental

Delta Dental PPO Plus Premier. Currently, Fluidnet pays 80% of dental insurance premiums.

• $50 deductible per individual/ $150 deductible per family. Deductibles do not apply to Preventive care.
• Services:

  For Premier Network (Premier Dentist): Preventive = 100% of eligible expenses Basic = 80% of eligible expenses Major = 50% of eligible expenses

  For Preferred Network (PPO Dentist): Preventive = 100% of eligible expenses Basic = 85% of eligible expenses Major = 55% of eligible expenses

Life and Disability Insurance

Currently, Fluidnet pays 100% of Life/AD&D and Short-Term/long-Term Disability Insurance premiums.

• Life/AD&D: 1 times salary to a maximum of $150,000.
